How to Make a Claim

If you are injured in a sanctioned game, practice or event make sure you start a claim by filling out the Insurance Claim Form.

- Complete the Insurance Claim Form in its entirety.

- Email the completed form to alainsurance@bflcanada.ca and CC programs@albertalacrosse.com within 30 days of the accident.


Insurance Certificate Requests

If you have been asked to provide a certificate of insurance for a facility you are renting for lacrosse purposes please complete and submit the Third Party Facility Insurance Request Form (coverage applicable in Canada only) to programs@albertalacrosse.com.

Insurance Certificates will be provided for the following:

- Any Club/LGB/member registered with the Alberta Lacrosse Association participating in a sanctioned event.

- Club/LGB/member must be up to date with their current fees and in good standing.
